The Bellarine Community Health Annual General Meeting (AGM) will be held on Wednesday 23 November. RSVP for this meeting in advance is essential.

Should you not be able to attend the AGM, you may nominate another person (proxy) to attend and vote on your behalf. Your proxy must RSVP to attend the meeting. The completed proxy form must be received by Kate Hughes, Chief of Staff, by no later than 5pm on Friday 18 November.
Proxy forms may be emailed directly to Kate.email@example.com; posted to PO Box 26, Point Lonsdale 3225 (please allow appropriate time for mail delivery); or hand delivered to the BCH offices at 2 Nelson Road, Point Lonsdale in an envelope clearly marked to Kate’s attention.

Please note that at the AGM, Dr Robert James’ term as a Board Director will expire in accordance with Section 11.3.2 of the Bellarine Community Health Constitution, “Elected Directors shall retire at the third AGM after their election”. Board Directors are eligible to re-nominate for election at the expiration of their term of office.
The decision has been made to conduct a Board Director election to replace this one (1) position at the AGM. Section 11.3.1 of the Constitution states “Nominations for the position of Elected Director at an Annual General Meeting may be submitted by a Financial Member”.
Financial members may nominate themselves or another financial member for the position of elected Board Director. If you plan to nominate another Financial Member, please ensure you have discussed it with that person and gained their acceptance of the nomination.
The Board Director Nomination form must be returned to Kate Hughes by no later than Monday 10 October.

The names and supporting information for each nominee will be circulated to Financial Members in the week beginning 7 November.
