Bellarine Community Health (BCH) is the major provider of primary health care services to the Bellarine Peninsula. BCH is a not-for-profit health care provider with close links to our community operating from five sites – in Drysdale (two sites), Ocean Grove, Point Lonsdale and Portarlington and in community settings across the Bellarine.

About the Team

The purpose of the Community Nursing and Palliative Care Team is to deliver holistic, value-based health care, in a collaborative manner to our diverse community. In accordance with our purpose, this position is responsible to facilitate a high-quality service provision that is responsive to clients’ need and empowers them to self-manage where appropriate.

About the role

The Community Nurse works autonomously within the community and is responsible for assisting and supporting clients in their home in the Bellarine area.  We receive referrals from a variety of providers, e.g Post Acute care, Hospital in the home (HITH), Commonwealth Home & Support Program (CHSP), Dept of veterans Affairs etc.  The Community Nurses also assist and support our Palliative care program clients.

BCH Nursing Service strives to achieve excellence through “Best practice” and evidence-based care to deliver holistic, value-based health care.

We would like to hear from you, if you…

  • Are a Registered Nurse (holding current registration with AHPRA).
  • Have at least 3 years’ post graduate nursing experience.
  • Have a demonstrated commitment to client-centred care and to best practice in community nursing and the Active Service Model approach to care.
  • Have experience in Hospital in the Home, Palliative Care and Post-Acute Care programs.
  • Have the ability to work autonomously while retaining the ability to work within a team setting.
