We are seeking an enthusiastic, energetic and skilled person to take on the role of Healthy and Connected Communities Planner. This Full Time position is available for commencement in January 2023.  The team is based at Point Lonsdale working onsite, remotely and in community settings.

Bellarine Community Health (BCH) is the major provider of primary health care services to the Bellarine Peninsula. BCH is a not-for-profit health care provider with close links to our community operating from five sites – in Drysdale (two sites), Ocean Grove, Point Lonsdale and Portarlington and in community settings across the Bellarine.

Our Healthy and Connected Communities team (HCCT) is a very passionate, friendly and highly skilled team, who aim to keep our community healthy and well and prevent people from getting sick in the first place. Our key focus areas are eat well, be active and tobacco-free living. We work closely with the local community, and regional and state bodies.

About the role

The Healthy and Connected Communities Planner will join a dedicated health promotion team and be responsible for undertaking primary prevention planning, implementation and evaluation as outlined within the organisations Health Promotion Plan – ‘Live Well Bellarine’.

This role will contribute to creating healthy, active, connected, self-reliant communities across the Bellarine Peninsula.

We would like to hear from you, if you…

  • Hold tertiary qualifications or substantial experience in Health Promotion, Health Science, Community Development, Education or Social Policy.
  • Have expertise in designing, planning, implementing and evaluating complex and multi-layered evidence-based health promotion programs and community development projects.
  • Are experienced in working with and implementing and facilitating youth or community led projects.
  • Have a passion for addressing health inequities and experience working in the areas of healthy eating with a climate and health lens would be an advantage and desirable.

Conditions of Employment

  • A current National Police Check (within 6 months from date of application)
