Bellarine Community Health is excited to offer this opportunity to join our current Home Care Packages (HCP)Team that will transition to Support at Home from 01 July 2025 Team as a Care Partner. This role will work in partnership with our SAH participants to provide an excellent customer service experience that optimises health and wellbeing, based across the beautiful Bellarine Peninsula! This Care Partner role is available to commence at minimum of 24 hrs per week (0.6FTE) with option to increase as case load increases.

About the team
The Support at Home Team works in partnership with participants, to achieve positive outcomes that optimise health and wellbeing in accordance with their needs, goals and preference, and to help them to live independently at home with safe access to their community.
About the role
The Care Partner, Support at Home will be responsible for the provision of care planning and case management for a caseload of older participants in receipt of Support at Home funding by:
- Providing ongoing comprehensive care planning to identify, implement and review the participants’ support plan and assist them to understand their approved services
- Assisting and enabling participants to understand their SAH funding in accordance with guidelines and compliance obligations
- Developing, managing and reviewing participant budgets on a quarterly basis and/or if there is a change in care needs
- Contribute to the ongoing development of the program including participating in development of procedures, planning and reviewing of participant services.
We would like to hear from you if you have:
- Highly developed care planning skills and the ability to creatively problem solve complex care situations.
- Proficient experience in case management and consumer directed care principles.
- An innovative and dynamic approach to the provision of care.
- Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and participants.
- Ability to build rapport and trust with participants and colleagues.
- Strong time management skills and the ability to prioritise complex workloads and tasks.
- Excellent computing skills including Microsoft Office and Excel.
- A Nursing or Allied Health degree qualification and APHRA registration as well as comparable experience would be highly regarded.
