Bellarine Community Health is excited to offer two Youth Community Health Nurse opportunities for experienced and qualified Registered Division 1 Nurses to join our dedicated Youth team. These roles are available 3 days per week (FTE 0.6) with a combination of ongoing and fixed term, with option of further fixed term.
These two Youth Community Health Nurses will join a dynamic, collaborative, and friendly multi-disciplinary team that provides physical, mental, medical, sexual, and social support for young people aged 12-25 and their family and support structures.

About the team
BCH’s Youth team provides a diverse range of primary health care services to our increasing population. Currently we deliver services across our sites including headspace Ocean Grove, BCH youth services, Doctors in Secondary School clinic, within schools and community settings. We are interested in hearing from nurses that can support this work and are interested in the wellbeing of young people.

About the Youth Community Health Nurse roles:
Working with adolescents, the Youth Community Health Nurse will deliver assessment, education and primary health care sessions to young people aged between 12 to 25 years as part of the multidisciplinary health team based at headspace Ocean Grove (or BCH youth site if required). Youth services outreach into schools and other community settings.
The Youth Community Health Nurse will provide critical support for the General Practitioner (GP) at the service sites, as well as providing the clinical link between the youth service, school, and other community settings. The nurse will liaise with clients, families, staff, and community, work with the GP, and make links with other allied health professionals working within the community to support a cohesive health management approach for young people.
Both roles are based at headspace Ocean Grove and some travel may be required.
We would like to hear from you if you have:
- Tertiary qualifications in Nursing and current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Experience in delivering evidence based therapeutic intervention, counselling & sexual health (or working towards qualifications).
- Demonstrated experience in mental health assessment and working with young people experiencing mental ill health.
- Demonstrated skills in solution focused practices.
- Strong time management skills and the ability to prioritise complex workloads and competing tasks.
- Excellent communication skills, both written and verbal.
- The ability to work in a dynamic and changing environment.
- An understanding of the principles of community health.
