Accessing Information held by BCH

An individual person has the right to access information from their own Medical Record by either obtaining copies or viewing their own Medical Record.

A person can obtain information from a deceased patient’s Medical Record if they were named next of kin at the time of admission, executor of the estate or are children of a deceased patient where both parents are deceased.

A Solicitor may apply for access on behalf of a client but must forward a recent consent signed by the client.

Medical reports – we may charge a fee for the service which does not exceed the amount of the person’s usual fee for a consultation of a comparable duration.

Our fees are set as per Freedom of Information (Access Charges) Regulations 2014

All requests for information under the Freedom of Information Act must be received in writing.

Information required by BCH

BCH is funded by both State and Federal Governments which means we are required to ask certain questions to meet their minimum data requirements and our reporting obligations.

Questions such as marital status and household living arrangements help establish if there is a combined family income or dependents. This information helps determine the cost of co-payment for services.

Gender questions are part of the Victorian Department of Health requirements and enable BCH to provide better healthcare that is clinically appropriate and inclusive.
