Bellarine Community Health is looking for an experienced and suitably qualified Intake Officer, to provide responsive service coordination through screening and referral management to the appropriate BCH clinical services. This role is currently available Part Time, with hours to be negotiated but likely to be around 0.6 FTE (3 days). Based in Point Lonsdale, this is an ongoing role.

About the team

Bellarine Community Health (BCH) is the major provider of primary health care services to the Bellarine Peninsula. BCH is a not-for-profit health care provider with close links to our community operating from six sites – in Drysdale (two sites), Ocean Grove (two sites), Point Lonsdale and Portarlington and in community settings across the Bellarine.

BCH delivers a broad scope of health services and wellbeing programs for all people in our diverse communities.

The Intake team are dedicated, professional, and caring and are committed to providing a high quality, personalised service based on client needs.

About the role

The Intake Officer role works as part of a dedicated and committed team to provide a professional, integrated, streamlined, single point of access to BCH services. This role is focused on ensuring continuous quality improvement, providing excellent levels of customer service, and accurate data collection.

To be successful you will require:

  • Experience in intake / service coordination role or similar, within a community setting or equivalent client services or other health related discipline.
  • Demonstrated advanced computer literacy skills inclusive of MS Office and client management systems.
  • Experience in referral management and referral screening.
  • Understanding of social determinates of health.
  • Demonstrated knowledge and commitment to continuous quality improvement.
  • Understanding of, and commitment to, the principles and values of BCH.
  • Excellent written and verbal communication skills.
  • Understanding of crisis intervention framework.
  • Ability to work both independently, and effectively and cooperatively within a team environment.
